Patches Australia Now Offering Patient Transport

Patches Australia is now offering Commonwealth Home Support Programme (CHSP) participants Patient Transport. This includes local pick up and drop off for social outings, medical and other appointments in and around the community.

To be eligible, participants must be CHSP funded for transport, and have a profile set up on the My Aged Care portal. All referrals must be submitted through this portal for transport requests to be accepted.

As Patches Australia recruits more drivers, more dates and times will become available.
